Changing Minds mental health and addiction inquiry survey!
The New Zealand Government has established an inquiry into mental health and addiction in NZ, this is due to increasing demand on health services and reports of services failing to meet the needs of people, their whānau, and people working in services.
Changing Minds is collecting feedback from people and whanau with lived experience of mental health and addiction to make a submission to the mental health and addiction inquiry panel.

There are other opportunities to make your own individual or organisational submissions to the mental health and addiction inquiry, info about this can be found here – www.mentalhealth.inquiry.govt.nz.
This survey is anonymous and no responses will be shared with anyone. The overall themes of the responses will be collated, evaluated and formed into a submission for the mental health and addiction inquiry.
Survey should take approximately 10 minutes to complete.
